Using the PDF form
This PDF may be filled out on your computer, then printed + signed, or it can be printed blank and filled out with a pen.
On a mobile device? You may be able to print the blank form, but we don't recommend trying to fill out the digital form unless you're on a desktop computer.
Delivering your Completed Forms
You may bring your completed forms with you on the day of your first appointment, which will speed up your appointment, or fax them to us ahead of time. Please phone us at 403-261-7661 to get our fax number.
Please do not scan + deliver your completed forms via email. It is not a secure enough platform for your medical information.
